Web application workflow system for surveying students to evaluate faculty. Integrates online creation and management of surveys with paper-based system.
Requesting surveys for an instructor and class begins the process. Survey requests are stored in the database and notifications are emailed to the requestor and survey administrator. If the section requested is online, an online survey is generated for students to complete. Otherwise, system prepares print outs to accompany paper forms.
Administrative system provides flexible search options for requestors to track the progress of their surveys and for administrators to manage survey lifecycle. System emails requestors and administrators when survey deadlines are reached.

Designed to minimize user input: only collect what cannot be derived, and even then provide most common defaults (to minimize the times a user would need to override):
- section number: populate class name, instructor name, and survey count (census enrollment counts becomes the survey form count)
- enforce business logic: only online classes receive online surveys

Surveys Search Form
Parametric search provides flexibility to find/browse surveys on single parameter or any combination thereof to fit the information needs of deans, requestors, and survey administrators.

Search results provide a heads-up view on the status of surveys and an interface to the details for each survey.
Survey Detail
Survey details groups related surveys and provides an interface to edit their values and perform various actions: generate survey cover letter, preview online survey, and export survey results.
Modifying Survey Administrators
Three levels of administrator privileges: primary, trusted, and technical. The primary administrator receives all new survey notifications and, serves as the primary contact for all respondents (all contact information inserted into surveys was theirs). Technical administrators receive notice of all surveys and error reports. Trusted users have privileges to create and manage surveys but cannot change administrative levels.
